needs to be renamed to pak0.pak in order to work?

Yes, it needs to.

I guess that depends on the engine, worked fine with requiem and the Injector for me.

requiem and the Injector worked. quakespasm and the Injector did not work.

I wasn't expecting to be fighting an actual 3d version of the Cyberdemon, a sprite version maybe (does Quake even support sprite monsters), but a full 3d model? Nice!

I think the author really nailed the behaviour of the cyberdemon. Maybe it could move just a tad bit more between the rocket volleys, but otherwise fantastic work on the monster. A lot of work must have been put on making the monster, and the rest of the geometry looks nicely faithful to the original map, too. The only thing missing are the lost souls, but it's okay, there must have been enough work with just the cyberdemon, as it is.

Overall, a solid 4/5 from me!

You can use quakespasm engine after unpacking the files from babel.pak to the c:\quake\babel directory. The quakespasm engine doesn't work with the archive babel.pak.
